On January 23, 2010, an employee at the baggage counter of Rustan's ShopWise store at Harrison Plaza, Manila, gave me a claim tag, which did not bear a visible indication of the location of said store. There are, as of January 24, 2010, several Rustan's ShopWise stores in Metro Manila. Without the indication on said claim
tag, I would have a hard time in proving that said tag was issued by said ShopWise store in Malate, Manila. Similar incidents occurred before and were reported to the
DTI.Also, again, an employee at said baggage counter accepted my luggage, which was locked, without inspecting first, the contents of said luggage.
Said luggage was large enough to contain deadly weapons.

warning vs. Aristocrat restaurant
at 432 San Andres st., 701 Malate, Manila. On December 30, 2009, I saw several employees at said restaurant, who did not have the required/recommended
health certificate displayed on the front of the clothing they were wearing. Said persons were identifed as “Elisa Constantino, cashier”, “Marcial Oladine,
supervisor”. “Awin, Team Leader”. Also, while I was eating, I saw a live moving insect fly on the rim of the drinking glass at the table I was using.
In addition, the take out food I ordered was placed inside what was described as a cardboard box together with a sauce container. The germs from said
sauce container could contaminate the exposed food. Also, said food was placed in direct contact with the said cardboard box. So the germs from said box
could contaminate the exposed food. Proof of purchase is the Aristocrat Restaurant Official Receipt numbered 117833 and dated Dec. 2009.

Said employee was identified as “Jocelyn".While I was at said store, I purchased food. Proof of purchase is said store’s official receipt dated 12/29/09

at store at Robinsons Ermita on December 28, 2009. Said employee had no health certificate attached to the clothing he/she was wearing, contrary to DOH recommendations/requirements. While I was at said store, I purchased food. Proof of purchase is said store’s official receipt dated 12/29/09 and numbered 0211969.Said employee was identified as “Regine”.

at Robinsons Ermita. On December 21, 2009, the receipt No. 3168 issued to me by an employee working at said store did not bear the address of said mall, but instead, bore the address: “HD Marketing & Distribution Phils, Inc. 2/F 203-204 A SM MALL OF ASIA, PASAY CITY.” So, if I had a complaint, I’d have to go to Pasay City. Is said establishment paying any taxes in Manila? I asked an employee working at said store to show me the sanitation permit which said store was supposed to have on display. Said employee didn’t show me said permit.
