Complaint vs. SMART Broadband, Inc. store at Powerplant Mall, Rockwell Center, Makati City.
On 05 September 2010, said store to me a SMART brand USB modem. I have proof of said sale. Said modem didn't work with Windows 7 operating system and with Windows XP operating system. While I was at said store, there was noprominently displayed information regarding the privileges of/for senior citizens. I believe the management of said

Donnie Tantoco of Rustan’s supermarket ignored customer’s complaint re security lapse at Rustan’s supermarket?
Said complaint was received on November 13, 2010 at Rustan’s supermarket at Shangri-la Plaza building in Mandaluyong. Up to March 28, 2011, said Donnie Tantoco has not contacted me regarding my said complaint.

On March 30, 2011, lax security at two street entrances to Robinsons Place Manila. At both street entrances, the guards at said entrances: didn't inspect my luggage/didn't use a metal detector to scan my luggage/didn't use a bomb sniffing animal to smell my luggage. Said luggage was large enough to contain fragmentation grenades.The first street entrance
The first street entrance was the street entrance to the Robinsons department store and the second street entrance was the street entrance beside the Robinsons supermarket.
After entering Robinsons, I went to the Canon store and bought a product. I have proof of payment.After entering Robinsons Place Manila the second time, I went to National book store and had some photocopying done. I have proof of payment from said store. While I was at said store, there was no visible prominently displayed information regarding the privileges of/for senior citizens. I believe the management of said store violated the implementing rules and regulations of R.A. 9257.

I have evidence of defective doorknob at said building. Said defective doorknob caused me inconvenience. For several minutes, I was unable to get out from what was described as a washroom at was identified as a food court in said building. Up to March 26, 2011, I have not received an apology for said defective doorknob.

Service (FSIS) encourages everyone to Be Food Safe and give your kitchen - especially refrigerators and freezers where raw
meat, poultry and seafood is stored - a thorough cleaning as well.

SpringWASHINGTON, March 24, 2010 - As you spring clean your closets, cars, and garages, the U.S. Department of Agriculture"s Food
Safety and Inspection Service (FSIS) encourages everyone to Be Food Safe and give your kitchen - especially refrigerators
and freezers where raw meat, poultry and seafood is stored - a thorough cleaning as well.

This is a good time of year to use or throw out items that are losing their quality or have spoiled, as well as to check
for unnoticed spills and remove lingering odors. Cleaning out your freezer requires extra care and can create new messes,
so FSIS is providing simple steps to help you spring clean your kitchen, prevent cross contamination, and reduce the risk
of foodborne illness.

Clean. Bacteria can be transferred by hands, cutting boards, and knives and quickly spread to all kitchen surfaces.
Frequent cleaning can keep that from happening.

Keep countertops clean by washing with hot soapy water before and after preparing food.
Keep the refrigerator clean at all times. Wipe up spills immediately and clean surfaces thoroughly with hot, soapy water,
and rinse them well.
If spoiled food has left an odor in your refrigerator or freezer as a result of a power outage, wash and sanitize shelves,
crispers, and ice trays, as well as the door and gasket. Leave the door open for about 15 minutes to allow free air circulation. FSIS has more tips on how to remove kitchen odors at www.fsis.usda.gov/PDF/

Sanitize surfaces and utensils with a solution of 1 tablespoon of unscented, liquid chlorine bleach per gallon of water.

Separate. Cross-contamination is the spread of bacteria from one surface to another, and it is especially likely to take
place when thawing or preparing raw meat, poultry, and seafood. Any bacteria that may be on frozen meat, poultry, and fish
can become active upon thawing and cause illness if food is not handled safely.

Keep fresh or frozen raw meats and any juices that may leak from them away from already-cooked food or fresh produce. Thaw
or store raw meat, poultry, and seafood in a container or on a plate in the refrigerator so juices can"t drip on other foods.
Use one cutting board for raw meat, poultry, and seafood. Use another for salads and ready-to-eat food.
Wash cutting boards with hot, soapy water after each use. Rinse with clear water and air dry or pat dry with clean paper
towels. Nonporous acrylic, plastic, or glass boards and solid wood boards can be washed in a dishwasher (laminated boards
may crack and split).
Replace cutting boards that are excessively worn or have developed hard-to-clean grooves where bacteria can live. Go to

Always use clean plates and utensils. Never place cooked food back on the same plate or cutting board that previously held
raw food.

Cook. Even for experienced cooks, the improper heating and preparation of food means illness-causing bacteria can survive.
Meat, poultry, and seafood should be cooked to a safe internal temperature to be sure bacteria that may be present is
destroyed.

Know the safe internal temperature for each dish you are preparing. Cuts of beef, veal, and lamb should be cooked to 145
°F; pork and ground beef should be cooked to 160 °F; and poultry should be cooked to 165 °F. A more complete list of safe
internal temperatures can be found at www.IsItDoneYet.gov.
Use a food thermometer to make sure food has reached the temperatures listed above - you can"t tell food is cooked safely
just by looking. To ensure the accuracy of the food thermometer, follow the package instructions or calibrate kitchen thermometers. To determine what type of food thermometer is best for your kitchen and your favorite recipes, visit www.fsis.usda.gov/Fact_Sheets/Kitchen_Thermometers.
After use, carefully wash food thermometers by hand with hot soapy water. Do not immerse them in water.
When microwaving, stir, rotate the dish, and cover food to prevent cold spots where bacteria can survive. If food spills in
the microwave, wipe it up immediately and clean surfaces thoroughly with hot, soapy water.

Chill. Bacteria grow fastest at temperatures between 40 °F - 140 °F, so chilling food properly is one of the most effective
ways to reduce the risk of foodborne illness.

Cool the fridge to 40 °F or below, and use an appliance thermometer to make sure the temperature does not rise.
Chill leftovers and takeout foods within 2 hours, making sure to divide food into shallow containers for rapid cooling.
Thaw meat, poultry, and seafood in the fridge, not on the counter, and don"t overstuff the fridge.
Once a week, make it a habit to throw out perishable foods that should no longer be eaten. A general rule of thumb for
refrigerator storage is 4 days for cooked leftovers; 3 to 5 day for raw steaks, roasts, and chops of red meat; and 1 to 2 days for raw poultry, ground meats, and fish. To see a detailed refrigerator storage chart, visit www.fsis.usda.gov/

Exposure from ionizing radiation following nuclear plant accidents may cause acute as well as long-term effects on health.

Health effects are largely determined by the degree and duration of exposure. The highest risk to health is found among
nuclear plant workers. Communities a few miles around the plant may also be affected. That is why evacuation maybe necessary to reduce this risk.

Very early occurrence of vomiting or very serious conditions or even death among exposed population, strongly suggest human
absorption of very high doses of ionizing radiation. Blood tests may be done to confirm exposure. Geiger counter or
dosimeter are also used to monitor progressive exposure to radiation.

Acute illness is known as acute radiation syndrome or radiation poisoning. This manifests as nausea, vomiting, fever or
diarrhea. More serious conditions appear with extreme exposure. A latent period follows and lasts for weeks or months. Then
more serious conditions affecting other organs may occur. Hair may fall and blood may be found in stools or vomit.

It is important that proper decontamination is done among those exposed. This means removing all clothes and shoes followed
by showering. Treatment is given to prevent organ damage.

Long-term effects after exposure include cancers and genetic malformations in fetuses.

Presently, Japanese authorities are implementing containment procedures to eliminate radiation leaks. People have also been
evacuated from a 12-mile radius. Hundreds have been screened for radiation exposure and so far no report of any illness was
made.


During a radiation emergency, such as fears of a nuclear plant explosion, you may be advised to create
a "shelter in place." This means you should stay inside your home or office, or perhaps another confined area indoors. To
keep your shelter in place effective, you should: close and lock all doors and windows; turn off fans, air conditioners, or
any units that bring in air from outside; move to an inner room or basement; keep your radio tuned to the emergency

DTI officials ignoring CSC re customer’s complaint vs. DTI?
I have a copies of two letters, both identified as from a source identified as Maria Luisa Salonga Agamata Director IV Public Assistance and Information Office of the CSC. The earlier letter was dated October 4, 2010 and was addressed to DTI Secretary Gregory L. Domingo regarding three of my letters sent to said DTI by registered mail but as of October 4, 2010 said DTI had not acknowledged receipt of said letters. Since, by December 12, 2010, no person from DTI had provided me with a reply to said Agamata’s October 4, 20 letter to said DTI Secretary, I informed the CSC about the my non receipt of a reply from said DTI re said Agamata’s October letter.





Dra. Vicky Belo ignored DTI letter?
I have a letter from a source identified as Victorio Mario A. Dimagiba Director Bureau of Trade Regulation and Consumer
Protection, 2/F Trade & Industry Building, 361 Senator Gil Puyat Avenue, Makati, and dated 19 October 2010 and addressed to
said Belo. Said letter contains the words: “...we refer to you for appropriate action (my) . . . attached complaint letter
of Mr. . . . He . . .in his letter. . . claimed that he has requested for information of a product which he sent through
registered mail and confirmed received by your agent months ago. However, to date . . . he has not yet been provided any
reply on the requested information.Kindly give this matter your most preferential attention and please inform the
complainant of the action to be taken by your office . . .” Up to March 14, 2011, neither said Belo or her agent has
provided me with a reply to said DTI letter.



Lax security on January 11, 2011, at entrance to Glorietta, Ayala Center, Makati
A guard, at a street level entrance to said building: didn’t inspect my luggage; didn’t subject said luggage to scanning
with metal detector; didn’t have a bomb sniffing animal smell said luggage. Said luggage was large enough to contain
grenades. Said security lapse occurred at the Glorietta entry point nearest the Mercury Drug store branch at said building.
After entering said building at said location, I bought a product at said drug store and was issued a Mercury Drug official
receipt numbered 941205.

at SM Mall of Asia, Pasay City
On February 19, 2011, I witnessed lax security and violations of R.A. 9257 at the SM Mall of Asia. Said area was
under the jurisdiction of Pasay City Mayor Calixto. Guards at two entrances
to said mall; didn’t inspect my luggage; didn’t use a metal detector to scan my luggage; didn’t have a bomb sniffing dog
smell said luggage. Said luggage was large enough to contain grenades. Several stores, in said mall,which are supposed to
obey the implementing rules and regulations of R.A. 9257, didn’t prominently display posters, etc. that generated public
awareness of the rights and privileges of senior citizens.. One of said stores was identified as Globe Broadband, Inc. I
have proof of purchase issued by said store.Rule IX Article 11 of Republic Act No. 9257 contains the following statement:
"It shall be the responsibility of the municipality/city through the Mayor to require all establishments covered by the Act
to prominently display posters, stickers, and other notices that will generate public awareness of the rights and
privileges of senior citizens. Based on said rule, it is the responsibility of Pasay City Mayor Calixto to require said
stores to prominently display said posters, etc.

FDA prompts removal of unapproved drugs from market
Class action is part of FDA’s Unapproved Drugs Initiative

The U.S. Food and Drug Administration today took action against companies that manufacture, distribute, or market certain unapproved prescription oral cough, cold, and allergy products. The affected products cannot be legally marketed in the United States.

Unapproved prescription cough, cold, and allergy drug products have not been evaluated by the FDA for safety, effectiveness, and quality. People may be at greater risk when using these products than when using FDA-approved prescription drugs or drugs that are appropriately marketed over-the-counter (OTC).

Many health care providers are unaware of the unapproved status of drugs and have continued to unknowingly prescribe them because the drugs’ labels do not disclose that they lack FDA approval.

“Removing these unapproved products from the market will reduce potential risks to consumers,” said Deborah Autor, director of the Office of Compliance in the FDA’s Center for Drug Evaluation and Research. “There are many FDA-approved prescription products, as well as appropriately marketed over-the-counter products, available to treat cough, cold, and allergy symptoms; so, we expect little or no negative impact on consumers from the removal of these unapproved products.”

Companies that have previously listed products subject to today’s action with FDA are expected to stop manufacturing them within 90 days and stop shipping the products within 180 days. Companies that have not previously listed products subject to today’s action with FDA are expected to stop manufacturing and shipping their products immediately.

Given that so many cough, cold, and allergy drug products exist that are FDA-approved prescription products or are appropriately marketed OTC, taking an unapproved prescription product is an unnecessary risk.

Consumers who believe they are taking an unapproved prescription cough, cold, or allergy product should contact their health care provider to discuss alternatives.

This is the 17th action on a drug class as part of FDA's Unapproved Drugs Initiative, which began in June 2006. The initiative is the agency’s risk-based enforcement approach to efficiently and rationally bring all unapproved new drugs into the approval process. One of the goals of the initiative is to reduce consumer exposure to drugs that are not proven safe, effective, and of high quality.

importer/distributor of said product. I believe the management of said store violated the Consumer Act of the Philippines. Also, on the days and times when I went to said store, there was no express lane for senior citizens and no required p

Mandaluyong Mayor tolerates Octagon store?
Said store was at SM Megamall and was identified as owned and operated by Proton Microsystems, Inc, Octagon Computer Superstore 747 Romualdez Corner Zobel Streets Manila. On 12/30/10, an employee at said store misstreated me, a senior citizen, Said employee issued to me a sales invoice No. 4289 where the handwriting below the following words in said invoice “Received the above items in good order and condition” is not my handwriting. Because of said handwriting in said space, I could no longer complain if I found something wrong with the product sold to me by said store. Also, while I was at said store on said date, there was no express lane for senior citizens in the customer area of said store and there was not prominently displayed in the customer area of said store information regarding privileges of/for senior citizens. I believe the management of said store violated the implementing rules and regulations of R.A. 9257. Based on said Act, it is the responsibility of the Mandaluyong Mayor to ensure the full implementation of said Act in Mandaluyong.

Shopwise Supercenter ignored DTI?

As a public service, am complaining vs. Shopwise Supercenter, which was at Harrison Plaza,Manila. I received a letter identified as from “Asst. Sec. MA. THERESA “Angel” L. PELAYO National Capital Region/Regional Operations Group and addressed to the BRANCH MANAGER Shopwise Center Harrison Plaza Mall A. Mabini cor. M. Adriatico Streets Malate .Said DTI letter is dated March 22, 2010 and contains the words: This refers to a public service advisory from Mr. MACARIO SILVA . . . relative to safety and security concerns in your establishment. We are referring to your establishment the herein attached public service advisory for your appropriate action . . .Action on the above subject may be communicated directly to the complainant . . .”. Up to Feb. 7, 2011, no one from Shopwise Supercenter has communicated with me regarding said DTI letter.


warning vs. Shopwise Supercenter
On February 11, 2010, the guard at the Shopwise Supercenter branch, in Harrison Plaza, Manila, closest to the Abenson store, didn’t inspect/subject to scanning my luggage which was large enough to contain deadly weapons. Said guard allowed me to bring said luggage into said store. Then, said store sold me, among others, what was identified as a picnic basket, swedish cinnamon bread, Kettle chips. Said picnic basket didn’t bear a visible (to unaided eye) name and address of the manufacturer/distributor of said product. Said swedish bread didn’t bear a visible, to unaided eye, address of the supplier of said product. Said chips, which were identified as an imported product, didn’t bear a visible, to unaided eye, name and address of the importer of said product.Proof of said sale is sales invoice SI No. 71 issued to me on February 11, 2010 by said store.While I was at said store, there was no visible, to unaided eye, express lane for senior citizens and while I was talking to a person at what was identified as the baggage counter of said store, said person attended to another person. I believe the implementing rules and regulations of the Expanded Senior Citizens Act of 2003 was violated. Said person issued to me a piece of plastic with a number and the name of said store but without the word “Harrison Plaza" or “Manila” on said piece of plastic. Since, as of said date, there were several branches of Shopwise in the Philippines, Without the word “Manila” on said piece of plastic, I would have a hard time proving that said goods were sold by said branch. When I asked said Shopwise employee to replace said piece of plastic with one with the word “Manila” on it, he replaced it with another piece of plastic also without the word “Manila”. Said issuance of claim tags at said store without the word “Manila” or “Harrison Plaza” on said tags occurred previously and I reported said wrongdoing to the Department of Trade and Industry. Based on evidence, said DTI wrote to said Shopwise branch regarding said wrongdoing and for a time said branch did not commit said wrongdoing. Apparently, said branch has resumed commiting said wrongdoing.
Said store was identified as owned & Operated by Rustan Supercenters, Inc. Donny Tantoco was identified as the head of Shopwise Supercenters.


I have complaint and evidence vs. Manila Vice Mayor Moreno.
Based on a letter dated April 7, 2010 and identified as from a source identified as Office of the Postmaster Central Post
Office Manila, registered letter No. 4972, addressed to said vice-Mayor, was delivered and received by Virgie . . . on
February 26, 2010. I was the person who sent said letter. Up to March 1, 2011, said Vice-Mayor has not provided me with
acknowledgement that he received said letter and has not informed me as to what he did with said letter. I believe said

Bautista above the law?

Based on evidence my registered letter addressed to said Quezon City Vice-Mayor Joy Belmonte was received by her agent on
7/16/2010. Up to March 1, 2011, said Belmonte has not provided me with a reply to said letter. I believe she violated
R.A. 6713.

I have a copy of a letter identified as from the BTRCP (Bureau of Trade Regulation and Consumer Protection 2/F Trade and
Industry Building, 361 Senator Gil J. Puyat Avenue, 1200 Makati City, Philippines) dated 18 August 2009 and addressed to
said Mayor regarding my letter regarding said store specifically for the required permits/licenses of said store. Said
letter contains the following words; “Information and assistance with the action taken thereon may be directly coordinated
to the above-mentioned complainant.” Up to March 3, 2011, said Mayor has not contacted me regarding said letter from said
BTRCP.

I sent a letter of complaint, via registered letter, to said F. Zobel de Ayala, regarding a defective doorknob at what was
identified as a washroom at the Glorietta building at Ayala Center, Makati. Said defective doorknob caused me discomfort.
For several minutes, I was unable to leave said washroom.Based on the information on the registry return receipt for said
letter, said letter was received by said F. Zobel de Ayala’s agent on 9/7/10. Up to March 3, 2011, said F. Zobel de Ayala
has not contacted me regarding my said letter.
