While I was in said store on said date, I purchased a product and was given official receipt No. BRE-1-000554569.

Lax security at Robinsons Place Manila on October 30, 2010. When I entered said building on said date, the guard, at a street entrance to said building, didn't
inspect the contents of my shoulder bag/didn't subject said bag to scanning with a metal detector. Said bag was large enough to contain a firearm. After entering said
building, I went to the Mercury Drug store. I bought two products and was issued an official receipt bearing the numbers 329991 dated October 30, 2010.

and I have proofs of purchases from said establishments.

When setting the trap, place a small amount of peanut butter (approximately the size of a pea) on the bait pan of the snap trap. Position the bait end of the trap next to the wall so it forms a "T" with the wall. Rodents prefer to run next to walls or other objects for safety and do not being out in the open.

In attics, basements, and crawlspaces and other areas that do not have regular human traffic, set traps in any area where there is evidence of frequent rodent activity. Some rodents, particularly rats, are very cautious and several days may pass before they approach the traps. Other rodents, such as house mice and deer mice, are less cautious and may be trapped more quickly.

We do not recommend using glue traps or live traps. These traps can scare mice that are caught live and cause them to urinate. Since their urine may contain germs, this may increase your risk of being exposed to diseases.

Also place traps in outbuildings and in areas that might likely serve as rodent shelters. Natural rodent predators, such as non-poisonous snakes, owls, and hawks, may also help control and reduce the number of rodents outside the home.

If you trap inside your home, but do not seal up rodent entry holes, new rodents will enter the dwelling. For information on how to seal up holes inside and outside the home, please see the Seal Up! page.

* Clean up any sources of food or water, and items that might provide shelter for rodents.
Clean up rodent food sources and nesting sites
various food containers with properly sealed lids

Keep food in thick plastic or metal containers with tight lids.

Prevent contact with rodents by cleaning up your home, workplace, or campsite.
Eliminate possible rodent food sources

* Keep food in thick plastic or metal containers with tight lids.
* Clean up spilled food right away and wash dishes and cooking utensils soon after use.
* Keep outside cooking areas and grills clean.
* Always put pet food away after use and do not leave pet-food or water bowls out overnight.
* Keep bird feeders away from the house and utilize squirrel guards to limit access to the feeder by squirrels and other rodents.
* Use a thick plastic or metal garbage can with a tight lid.
* Keep compost bins as far away from the house as possible (100 feet or more is best).
* Keep grains and animal feed in thick plastic or metal containers with tight lids. In the evening, uneaten animal feed should be returned to containers with lids.

If storing trash and food waste inside the home, do so in rodent-proof containers, and frequently clean the containers with soap and water. Dispose of trash and garbage on a frequent and regular basis, and pick up or eliminate clutter.

Eliminate possible nesting sites outside the home. Elevate hay, woodpiles, and garbage cans at least 1 foot off the ground. Move woodpiles far away from the house (100 feet or more is best). Get rid of old trucks, cars, and old tires that mice and rats could use as homes. Keep grass cut short and shrubbery within 100 feet of the home well trimmed.

Got Mice? Seal, Trap, and Clean Up to Control Rodents
http://www.cdc.gov/Features/Rodents/

Rodents may enter your home for food or shelter. Seal up holes or gaps in your home, trap any existing rodents, and
clean up any sources of food or water and items that might provide shelter for them.

Mice and rats are pesky critters that can enter your home through small holes or gaps. Mice can squeeze through a hole the size of a nickel, and rats can squeeze through a hole the size of a half quarter!

Worldwide, rats and mice are the cause of over 35 diseases! In the United States, rodents can spread diseases like hantavirus pulmonary syndrome, rat-bite fever, leptospirosis, and lymphocytic choriomeningitis virus, a virus that poses a particular risk for pregnant women. If rodents invade your home this fall or winter, here are a few steps to protect yourself and your family.
Rats! What To Do about Rodents!

If you find signs of rodents or their droppings in your home, take precautions to clean up the area safely.

* Seal up holes or gaps in your home to prevent rodents from returning.
Mice can squeeze through a hole the size of a nickel, and rats can squeeze through a hole the size of a half dollar! Prevent rodents from entering the home by checking inside and outside the house for gaps or holes.
Where to look for gaps or holes inside your home

* Inside, under, and behind kitchen cabinets, refrigerators and stoves.
* Inside closets near the floor corners.
* Around the fireplace.
* Around doors.
* Around the pipes under sinks and washing machines.
* Around the pipes going to hot water heaters and furnaces.
* Around floor vents and dryer vents.
* Inside the attic.
* In the basement or crawl space.
* In the basement and laundry room floor drains.
* Between the floor and wall juncture.

Where to look for gaps or holes outside your home

* In the roof among the rafters, gables, and eaves.
* Around windows.
* Around doors.
* Around the foundation
* Attic vents and crawl space vents.
* Under doors.
* Around holes for electrical, plumbing, cable, and gas lines.

Fill small holes with steel wool. Put caulk around the steel wool to keep it in place. Use lath screen or lath metal, cement, hardware cloth, or metal sheeting to fix large holes. These materials can be found at your local hardware store. Fix gaps in trailer skirtings and use flashing around the base of the house. If you do not remember to seal up entry holes in your home, rodents will continue to get inside. Outbuildings and garages should also be sealed to prevent the entrance of rodents.

Public service advisory re Rustans stores in Makati, Manila, Mandaluyong. When I wrote to said stores asking for proofs, if any, that cosmetics they sell really contain the products they were advertised to contain and provide the benefits they were advertised as allegedly providing, said stores either did not provide me with a reply, or referred me somewhere else, or provided me with a brochure regarding said product. Security lapse on or about July 18, 2010, at entrance to Rustans store at Ayala Center, Makati.The guard at said entrance didn't inspect my luggage or subject my luggage to scanning with metal detector. Said luggage was large enough to contain grenades and firearms. Then the employee, at what was identified as the package counter of said store, accepted said luggage without first inspecting said luggage or subjecting said luggage to scanning with metal detector. I have a piece of paper said employee signed as proof he received said luggage.

Learn to Stay Safe from Home Fires
http://www.cdc.gov/Features/FirePrevention/

Learn how you can make your home a safer place for you and your family. You can prevent home fires.
The Risk is Real

The risk of injury and death from home fires is real.

Keep Your Home Fire Safe

Your home should be a safe and comfortable place—and you can help keep it fire-safe by following these prevention tips.

Cook with care. When you cook, never leave cooking food unattended on the stove. Keep anything that can catch on fire, like potholders and towels, away from the cooking area. Avoid wearing clothes with long, loose-fitting sleeves that can catch on fire. Also, keep pot handles turned in.

If you smoke, attempt to quit. Don't smoke inside your home. If you do smoke in your home, never smoke in bed or leave burning cigarettes unattended. It is unsafe to smoke while drowsy or under the influence of alcohol or medications. Also, don't empty burning or hot ashes in a trash can, and keep ashtrays away from upholstered furniture and curtains.

Protect the Ones You Love
In an effort to raise parents' awareness about the leading causes of child injury—including burns— in the United States and how they can be prevented, CDC launched
the "Protect the Ones You Love" initiative. Parents can play a life-saving role in protecting children from injuries. Information is available in English and Spanish.
Learn more.

Stay warm—safely. If and when you use a space heater, keep it more than three feet away from anything that can catch on fire, like draperies.

Be alarmed. Install smoke alarms on every floor of your home, including the basement, and make sure you have smoke alarms near all sleeping rooms. For better protection, install smoke alarms in sleeping rooms, especially if they are occupied by a smoker. Test all smoke alarms once a month using the test button.

Make and practice an escape plan. Create a home fire escape plan. Know at least two ways out of every room, if possible, and have a meeting place outside. Practice
your escape plan twice a year with everyone living in your home.

For more tips on protecting children from burns, visit CDC's Protect the Ones You Love.
A Time to Act

An online video available through CDC-TV, "A Time to Act", chronicles the experience of a family that escaped during a home fire after being alerted by recently installed smoke alarms. The video also reviews the main risk factors for home fires and steps viewers can take to reduce the risk of fire-related injury and death. These steps include installing and regularly testing smoke alarms and practicing a fire escape plan at least twice a year.

"A Time to Act" is available for viewing through a number of channels including mobile phone, or as a podcast, or for download (see "Download this Video" instructions on the CDC-TV page) thus allowing organizations to utilize our online video in any number of ways including as an instructional tool for parents, healthcare providers or teachers. Providing short, high-quality videos is part of CDC's goal to increase people's access to the information necessary to help prevent illness, injury and to protect their health and that of their families.

U.S. FDA warning letter to Johnson & Johnson

WARNING LETTER

HAND DELIVERED

Mark Bowden
Vice President of Global Regulatory Affairs
Johnson and Johnson Consumer Products, Inc.
199 Grandview Road
Skillman, New Jersey 08558-1303

10-NWJ-17

Dear Mr. Bowden:

This letter is in reference to the Listerine Total Care Anticavity Mouthwash distributed by your firm. The label for this product makes the following claims: "Strengthens Teeth, Restores Minerals to Enamel, Fights Unsightly Plaque Above the Gum Line, Helps Prevent Cavities, Kills Bad Breath Germs, and Freshens Breath."

Based on these labeled claims "Fights Unsightly Plaque Above the Gum Line" and "Prevents Cavities," Listerine Total Care Anticavity Mouthwash is a drug as defined in section 201(g) of the Federal Food, Drug, and Cosmetic Act (Act) (21 U.S.C. § 321(g)) because the product is intended for use in preventing or mitigating disease, or to affect the structure or function of the body, by preventing cavities and removing plaque. Sodium fluoride 0.0221% (0.01% w/v fluoride ion) for the purpose of "Anticavity" is the sole active ingredient listed for this product. This product is subject to the Final sole active ingredient listed for this product. This product is subject to the Final Monograph for Anticaries Drug Products for Over-the-Counter Use, 21 CFR Part 355, which covers the product's disease claim of helping "prevent cavities," and includes this active ingredient.

However, another claim on the label of Listerine Total Care Anticavity Mouthwash is "Fights Unsightly Plaque Above the Gum Line." This statement represents that the product fights plaque, a well-known precursor to gum disease, including gingivitis. Antiplaque/antigingivitis claims are not covered by the Anticaries Final Monograph. Such antiplaque/antigingivitis claims are, however, addressed in the Advanced Notice of Proposed Rulemaking (ANPR) for Oral Healthcare Products for antigingivitis/antiplaque (68 Fed. Reg. 32232 (May 29, 2003)). In that ANPR, the agency identified active ingredients under consideration for inclusion in an antigingivitis/antiplaque monograph; however, sodium fluoride is not among them.

Thus, no mouthwash with sodium fluoride as the active ingredient has been included or proposed for inclusion in any monograph for the antiplaque/antigingivitis indications claimed for Listerine Total Anti-cavity Mouthwash; that ingredient is not included among those under evaluation in the ANPR for antiplaque/antigingivitis drug products, nor does the anticaries monograph at 21 CFR part 355, which does include the active ingredient of sodium fluoride, include antiplaque claims as recognized claims. As formulated and labeled, Listerine Total Anticavity Mouthwash is not generally recognized as safe and effective for the antiplaque indications in its labeling, and it is, therefore, a new drug under section 201(P) of the Act (21 U.S.C. § 321 (P)). Under section 505(a) of the Act (21 U.S.C. § 355(a)), a new drug may not be introduced or delivered for introduction into interstate commerce unless it is the subject of an FDA-approved application. The marketing of Listerine Total Anticavity Mouthwash without an FDA-approved application violates this provision of the Act.

In addition, the front panel of your product states the product name: "Listerine Total Care Anticavity Mouthwash." The violative claims described in the above paragraphs, combined with the appearance of the "Total Care" name, suggests that the product is comprehensive in function, and will provide benefits, including antigingivitis and antiplaque benefits. We are not aware of any support for the antiplaque/antigingivitis claims or other statements suggesting that the product is comprehensive in function, providing benefits beyond those related to prevention of cavities. Thus, the product's labeling claim that it will provide all of the benefits listed, is misleading and accordingly makes it misbranded within the meaning of section 502(a) of the Act. (21 U.S.C. §352(a)).

The violations cited in this letter are not an all-inclusive list of deficiencies. You are responsible for investigating and determining the causes of these violations and for preventing their recurrence and the occurrence of other violations. You are to assure that your firm complies with all requirements of federal law and FDA regulations.

You should take prompt action to correct the violations cited in this letter. Failure to promptly correct these violations may result in legal action without further notice, including, without limitation, seizure and injunction. Other federal agencies may take this Warning Letter into account when considering the award of contracts.

We note additionally that your principal display panel (PDP) describes your product as "Sodium Fluoride and Acidulated Phosphate Topical Solution." The Drug Facts panel lists the sole active ingredient as "Sodium fluoride 0.0221% (0.01% fluoride ion)." These representations are inconsistent. Based on the information in your Drug Facts panel, it appears that the PDP should refer to either "Sodium Fluoride Acidulated Phosphate Solution" or just "Sodium Fluoride Solution." The Sodium fluoride listed in the drug facts panel should be listed at 0.02%; not 0.0221 %.

Within fifteen working days of receipt of this letter, please notify this office in writing of the specific steps that you have taken to correct the referenced violations. Include an explanation of each step being taken to prevent the recurrence of violations, as well as copies of related documentation. If you cannot complete corrective action within fifteen working days, state the reason for the delay and the time within which you will complete the correction. Your reply should be addressed to Robert J. Maffei, Compliance Officer, at the above address.

Sincerely,

/s/

DIANA AMADOR-TORO

Sprouts: What You Should Know
http://www.foodsafety.gov/keep/types/fruits/sprouts.htmlDo sprouts carry a risk of illness? Like any fresh produce that is consumed raw or lightly cooked, sprouts carry a risk of foodborne illness. Unlike other fresh produce, seeds and beans need warm and humid conditions to sprout and grow. These conditions are also ideal for the growth of bacteria, including Salmonella, Listeria, and E. coli.

On September 30, 2010, while I was at Pancake House, Inc. at Level 4, Robinsons Place, Ermita, Manila, I didn't see required express lane for senior citizens in the customer area of said restaurant. I also didn't see, in the customer area of said restaurant, sign, etc. regarding privileges of senior citizens. Front line employee at said restaurant didn't have health certificate attached in the proper position on the clothing she was wearing. Proof of purchase is said restaurant's official receipt No. 1761 dated Sept. 30, 2010.

Bureau of Foods and Drugs ignored referral letter from DTI?

Based on evidence, a " Asst. Sec, Ma.Theresa "Angel" Pelayo National Capital Region Regional Operations Group" wrote a referral letter addressed to DIR Leticia
Barbara B. Gutierrez, of said bureau and dated December 23, 2008. Said letter contained the following words: ". . . referred . . . the herein attached two (2)
advisories from . . . M. Silva . . . relative to personal care creams, as the subject items/products fall within the jurisdiction that office. Action on the above
subject may be directly communicated to the complainant . . ." Up to October 3, 2010, said Bureau has not communicated with me regarding said advisories.

customer advisory vs. Star Appliance Center, Inc.
which was at SM Appliance Center, SM Makati Annex Bldg., Ayala Center, Makati.
On April 08, 2010, said firm required me to pay in advance, Philippine pesos 500, for installation by Ramecon Industries Corporation, of a room type air conditioner.
Proof of said payment is a copy of SM Appliance Center sales invoice with the above date and numbered 12251. The installation by said Ramecon Industries was not done
properly. For approximately 14 days after said installation, every time I turned on said air conditioner, excessive sound emanated from the vicinity of said air conditioner. The address of said Ramecon Industries Corporation was: 2300 Aurora Blvd., Pasay City.

St. Luke's Medical Center ignored DTI?
